New Patient Medical
Everyone registering with the practice must provide evidence of their:
identity (e.g. passport, birth certificate or photo card driving licence)
address (e.g. a utility bill)
You will be asked to complete a registration form and a new patient questionnaire, allowing us to provide medical care in the interim period while your medical records are transferred from your old practice to us. You will also be given an appointment for a new patient medical with our practice nurse and you need to bring any medications along with you when you attend for your medical.
Well Baby and Immunisation
Health Visitor led clinics supervised by the GP's are held every Wednesday morning from 10.00am to 11.00am. This clinic is for child immunisations, vaccinations, weight checks, developmental screening and general reviews.
There is a midwife here every Tuesday morning for ante-natal clinic from 9:30am to 11:00am. This clinic is by appointment only.
Our practice nurse holds a variety of clinics including Asthma, Heart Disease Prevention, Hypertension (blood pressure), Diabetes, Family Planning advice, Cervical Screening, Smoking Cessation, Well person checks, Weight Monitoring and Vaccinations &Immunisations. These clinics are available by appointment only. Patients who have not been seen by a clinician at the practice for three years will be offered an appointment for a well person check up.
The District Nurses are based at Altcross Clinic they deal with chronic disease management for housebound patients. Dressings, suture removal, ear syringing etc are dealt with at the Treatments Rooms and patients need to make an appointment.
There is a Health Trainer attached to the practice, who can advise patients on all aspects of general health such as diet, exercise, stopping smoking, how to manage stress and anxiety, managing debt and where appropriate he can refer patients to a variety of outside agencies. The Health Trainer sees patients in the practice on Thursday mornings by appointment only.
